1. I need a description of the various boxes on my invoice.

2. What is the definition of "Anniversary Date?"

The Anniversary Date is the date that the service requirements are completed for the activation of your service. Regular billing will begin on this date.


3. What are the billing periods for each service?

Some services are billed quarterly and some monthly. Some services are billed in advance and some are billed in arrears which allows determination of usage for the previous period. Here's the breakdown:
